Mission Statement

The Thomas Jefferson Area Coalition for the Homeless aims to make homelessness rare, brief and nonrecurring in our community.

Overview

TJACH serves as the lead agency on homelessness in our region:
• setting local policy on homelessness through the development and implementation of a community plan to end homelessness
• incubating improved coordination and collaboration within the homelessness system of care through incentives and feedback
• distributing and monitoring local, state and federal funding to local providers in the interest of meeting the goals of the community plan
• educating providers and the general community on homelessness
• gathering and reporting on community level data

Key Programs

Program 1
Title:Service Provider Council
Description:The Service Provider Council, a committee of the TJACH Governance Board, provides a monthly opportunity for service providers to share information, learn best practice strategies, track community and program level changes, review data and implement the strategies of the Community Plan to End Homelessness.
Examples of Program Success:Through regular communication and the identification of shared values, local providers have created new collaborative efforts to address and end homelessness.

One such effort is the Community Case Review which meets twice per month to problem-solve and case-plan jointly so that people experiencing homelessness get access to all the resources available to them. Providers from 8 different agencies join forces to tackle difficult situations and share resources.
Program 2
Title:Community Plan to End Homelessness
Description:TJACH's work is directed by a formally adopted Community Plan to End Homelessness. The plan focuses on solving homelessness through housing and includes specific, measurable, and achievable annual goals:
• Reduce the number of people experiencing homelessness by 20%
• Reduce the amount of time people experience homelessness by 20%
• Increase the number of people exiting homeless service programs to permanent housing by 40%
• Increase the number of people exiting homelessness service programs with improved sources of income by 25%
• Decrease the number of people that return to homelessness within 12 months of program exit by 20%
• Increase the number of people that remain in housing for six months or longer to 80% or more
Examples of Program Success:By focusing on the hardest to serve, the most vulnerable, dedicated homelessness service provides have reduced homelessness across all subpopulations with the most significant impact being a 43% reduction in chronically homeless households over the last 5 years!

Program 3
Title:Data & Evaluation Committee
Description:The Data & Evaluation Committee, a committee of the TJACH Governance Board, collects and reviews data to ensure that our goals are being met. To that end, the committee has created a formal monitoring protocol to assess the performance of funded agencies working to end homelessness.

Building on themes from 10 discussion sessions with 67 community members, and several leading national frameworks for nonprofit health and impact, the 7 Actionable Principles aim to create a shared understanding of what makes a healthy nonprofit and build a supportive learning community of nonprofits and funders.
