The PACT team is seeking a dynamic and energetic nurse (RN or LPN) looking to work with individuals with an SMI diagnosis.  This unique position offers the opportunity to work with a multi-disciplinary team of experienced professionals.  Incumbent must be willing to engage with consumers in a variety of settings.  The hours of this position are flexible with some weekend hours required.  Associate’s or Bachelor’s degree in Nursing required.  Experience in working with individuals with SMI preferred.  LPN candidates must also meet QMHP or QMHP-E requirements.  Salary negotiable.

Nurse, Intensive Community Treatment, Nelson County

As a member of the new ICT Team in Nelson, this Nurse position is responsible for conducting psychiatric assessments, assessing physical health needs, and making appropriate referrals to community physicians. The incumbent will provide management and administration of medication in conjunction with the psychiatrist and will share in shift-management responsibility with other staff.  Must be Registered Nurse with bachelor’s degree and one year of experience or Licensed Practical Nurse with three years of experience. Certification as QMHP Adult and a VA driver’s license plus an acceptable driving record are required. Salary negotiable.
