Case Manager/Options Counselor Full time

JABA is seeking someone who is enthusiastic about helping older adults and adults with disabilities to identify resources and to coordinate services to support aging in the place of their choosing. This full time position is serving based in Albemarle county but serves the counties of Fluvanna and Greene (part-time at each location). This position requires significant local travel. Candidate must work well within a team environment as well as work independently. The candidate must be willing to develop strong relationships internally and with other community partners, in order to serve as an advocate for senior needs.  Requires knowledge of local and governmental policies and procedure, human services resources, aging issues, casework principles, and person centered planning. Proven ability to use computer to access internet, use applications including MS Office Word, Excel and Outlook, and online data management system to document services.  Must be able to lift 10 lbs., climb stairs, walk, stand, stoop/bend, and perform repetitive hand motions.
